used standard text, ref A, except for the specification of number
and date above.
3. Post pouched certified copy of the U.S. note and attachment, and
original of the GOJ DNS-IN document, to the Department (ISN/NESS -
Robin DeLaBarre) December 18, 2009 under registry no. 5723938.
